Internal USB connections to header? What goes where?

I have an internal header cable to go from my MX4 to my USB2 header on my motherboard.

The motherboard has the following connections shown:-
USB+5v
USB-5v
LP5+
LP5-
LP4+
LP4-
GND
NC


The cable has:-
Ground
+5v
+D
-D


What goes to where?

What type of motherboard do you have? Can you give me the link for the datasheet?

ok, the header is

(+5)(LP5-)(LP5+)(GND)(NC)
(+5)(LP4-)(LP4+)(GND)

Connect to one row, top or bottom...

+5v => +5
+D => LP4+
-D => LP4-
Ground => GND

Thanks... So I'll just connect to the pins along the bottom then...

That'll be perfect, just make sure you connect each pin as Henry outlined!! :)
